Overview
An exciting opportunity has arisen within the Helensburgh and Lomond area for a 30 hour Band 5 Community Discharge Liaison Nurse.
The post will be part of the Integrated Care Assessment Team working to facilitate patient discharges from hospital to community setting and preventing hospital admissions.
Responsibilities
Working within the scope of professional practice, provide clinical skills in the daily management of transfer of patients to the community setting.
Qualifications and Experience
You must be a RN (ADULT) with current NMC registration.
You should be able to demonstrate evidence of continuing professional development and be able to work within a multi-disciplinary team setting.
Previous Community Experience Is Preferable But Not Essential.
