Job Overview

The Team Leader position is a crucial role in overseeing the Transport and Social Support Individual (SSI) programs. Our ideal candidate will have experience in planning and scheduling, client record management, and evaluating service delivery.


Responsibilities

* Ensure compliance with government guidelines for SSI and Community Transport programs.
* Plan and schedule program activities, maintain accurate records, and assess service delivery outcomes.
* Cultivate working knowledge of the My Aged Care portal and relevant community resources.
* Promote effective communication skills, work with diverse clients, and demonstrate expertise in not-for-profit organizations.


Requirements

* A proven track record in team leadership or similar roles.
* Strong planning, organizational, and problem-solving skills.
* Excellent written and verbal communication abilities.
* Familiarity with the My Aged Care system and local community services.
