Job Title: Case Manager

* Bachelor's degree in Nursing or related field
* Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM)
* Three years' recent clinical nursing experience in Home Health and case management or an equivalent combination of education, training and experience
Responsibilities:
* Assess client needs and develop individualized care plans
* Coordinate services and resources to meet client needs
* Monitor client progress and adjust care plans as needed
* Collaborate with healthcare teams to ensure seamless care delivery
