THE ROLE
Reporting to the Clinical Lead, the Support Coordinator will manage a caseload of NDIS participants and support them to implement their NDIS plans across informal, mainstream, community and funded supports.
This is an outcomes-focused role working with participants with complex needs, including mental health and psychosocial disability. Home visits are a key component of this position.
You will support participants to build capacity, strengthen independence, and achieve the goals outlined in their NDIS plans while meeting NDIA reporting, compliance, and quality standards.
KEY RESPONSIBILITIES
* Deliver Support Coordination in line with NDIS Practice Standards
* Provide person-centred, strengths-based support to participants
* Develop and implement service plans to address barriers and risks
* Coordinate and negotiate appropriate supports with multiple stakeholders
* Support participants to maximise outcomes within available NDIS funding
* Liaise with Plan Managers, NDIA, NDIS Quality & Safeguards Commission, and service providers
* Maintain accurate, high-quality, and compliant participant records
* Complete mandatory reporting and assessments within required timeframes
* Conduct home visits and community-based work as required
SKILLS & EXPERIENCE
* Proven experience delivering NDIS Support Coordination
* Strong working knowledge of the NDIS system, Practice Standards and reforms
* Experience supporting participants with complex psychosocial disability and mental health needs
* Demonstrated skills in assessment, planning, referrals and case management
* Ability to work autonomously in a fast-paced, KPI-driven environment
* Excellent written and verbal communication skills
* Strong time management, organisation, and problem-solving skills
* Knowledge of local specialist and mainstream service providers
ESSENTIAL REQUIREMENTS
* Relevant tertiary qualification in Psychology or Social Work
* Right to work in Australia
* Ability to obtain and maintain:
* National Police Clearance
* Working With Children Check
* NDIS Worker Screening Check
