At Your Journey Your Choice Support Services, we are currently seeking both male and female Support Workers to join our growing team. We are looking for dedicated, reliable, and compassionate individuals who are available to work across a variety of locations between Brisbane and Tweed Heads.

As a Support Worker, you will play a vital role in supporting people living with disabilities and mental health challenges to live more independently, build confidence, and achieve their personal goals. This role is about far more than providing care — you will become a mentor, motivator, advocate, someone who can solve problems/problem solving skills and positive influence in the lives of the participants you support.

We are looking for people who are organised, approachable, respectful, and able to build strong professional relationships while maintaining clear boundaries. If you are someone who naturally encourages others, remains non-judgemental, and enjoys making a genuine difference in people’s lives, this could be the perfect opportunity for you.

This is a dynamic and rewarding position where no two days are the same. You will provide both practical and emotional support tailored to each participant’s individual needs, helping them develop skills, increase independence, and actively participate in their community.

Key Responsibilities
Supporting Daily Living & Independence
* Assist participants in developing essential life skills such as budgeting, meal preparation, healthy living, personal organisation, and using public transport.

* Encourage independence by providing guidance, structure, and support that builds confidence and self-reliance.

Skill Building & Personal Development
* Support participants to explore hobbies, interests, and community activities that promote personal growth and social inclusion.

* Assist participants in accessing appropriate services, resources, and funding opportunities to achieve their goals.

Personal Care & Practical Support
* Provide assistance with personal care, medication administration, household tasks, transport, and daily routines as required.

* Maintain a safe and supportive environment by identifying risks and following appropriate safety procedures.

* Work collaboratively with families, support networks, and healthcare providers to ensure consistent, high-quality care.

Emotional & Social Support
* Build positive and meaningful relationships with participants through encouragement, active listening, and emotional support.

* Support participants to engage in social, recreational, and community-based activities that improve wellbeing and quality of life.

Healthcare & Documentation
* Assist participants with attending appointments, medication checks, and maintaining accurate healthcare records and progress notes.

* Support participants to better understand and manage their healthcare needs.

Team Collaboration
* Work closely with coordinators, allied health professionals, families, and other support staff to deliver person-centred support.

* Follow individual support plans, behaviour support strategies, policies, and best practice guidelines to ensure high standards of care are maintained.

Skills Required:

* Empathetic Communication: You listen actively, with both your ears and your eyes, to fully understand participants’ needs.

* Respectful Approach: You have a soft, empathetic communication style that ensures participants feel valued and respected.

* Emotional Resilience and Patience: This role requires a calm demeanour, patience, and the ability to provide encouragement.

* Adaptability: You’ll need to adjust to different situations quickly, working independently or as part of a team as needed.

* Relationship-Building: Strong interpersonal skills and a positive approach to building connections with participants and their families.

* Organizational Skills: Efficient time management and thorough record-keeping ensure that nothing slips through the cracks.

* Certificate IV in Community Service/Disability Services or equivalent qualification with equivalent experience in the disability or mental health, OOHC sectors.

* Must have a driver's licence and reliable transport

* Roadworthy and Insured Vehicle

* Current First Aid & CPR-valid.

* National Police Clearance within last 12 months.

* NDIS 'Worker Orientation' module

* Proof of Australian working rights

* Current paid employee Blue Card

* Current paid employee Yellow Card/NDIS Worker Clearance.

* Smart phone with internet access

* Two current (within the past 2 years) professional references. One of those references will be required from a Supervisor/ Management role.
