About the role:
is a geriatrician-led multidisciplinary service, offering care to the older person, with complex medical, functional and or psychosocial needs, in their own home. Community Geriatrics provides short-term case management to prevent hospital admissions, through the delivery of care in the home environment.
Reporting Operationally to the Advanced Nurse Unit Manager, some of your duties for the role will include:
* Providing services to community-dwelling patients via multiple modalities, including but not limited to face-to-face and Telehealth
* Be required in a multidisciplinary primary health care setting to apply clinical expertise to assess clients, select and implement different therapeutic interventions and/or support programs, and evaluate patient/client progress
* Linking clients and carers with relevant community resources and services
* Communicating with the client's family, carers and other service providers
* Providing leadership to the Community Geriatrics team, including supporting recruitment, performance management and development, rostering, allocating tasks, and other human resource management
* Mentoring and supporting other professional members of the team
About you:
* Registered Nurse with at least 3 years, full-time equivalent, post-registration experience or an Allied Health professional holding an appropriate degree level qualification and membership (or eligibility for membership) with the relevant professional association, and or registration with the relevant Board
* Experience in the supervision of students, and less experienced staff
* Effective leadership skills, including highly developed skills in communication, problem solving, conflict resolution, and negotiation skills
* Clinical experience as a senior practitioner within a subacute setting, including clinical experience with complex patients
* Recent experience working in a specialist geriatric unit

This is a single vacancy, with two possible appointment classifications. The successful applicants will be appointed in the position appropriate to their skills, qualifications, and experience.
Please note that this position is multi-classed as either a AHP3 or RN3 and classification will be determined by qualifications and experience.
