Job Opportunity: Clinical Nurse/Senior Health Practitioner
This role involves undertaking complex assessments of aged clients in various settings, including community and hospital environments. The primary objective is to record accurate data collected during these assessments using the Integrated Assessment Tool (IAT) within the Aged Care Assessor application or My Aged Care portal.
About the Role
As a clinical assessor, you will develop client-centered support plans detailing recommendations for aged care and community pathways. This will enhance service provision and agreed outcomes. You will provide up-to-date information on suitable aged care pathways for eligible clients and liaise with clients, carers, health professionals, service providers, and other internal and external stakeholders.
Team Overview
The Aged Care Assessment Team (ACAT) is transitioning to the Single Aged Care Assessment System. This small, highly skilled multidisciplinary team provides assessment services to older Queenslanders in the community. The main purpose is to simplify and improve access to aged care services, helping clients maintain their independence at home or enter residential aged care settings.
