Care Coordination is a critical function that plays a vital role in delivering exceptional care to individuals. As a Clinical Care Coordinator, you will be responsible for leading clinical and care teams to ensure seamless care delivery.
Key Responsibilities:
* Ensure comprehensive clinical and specialized nursing care is provided to residents by our clinical staff.
* Prioritize residents and their families in decision-making, considering their preferences, independence, dignity, and identified care-related risks.
* Oversee and conduct all scheduled AN-ACC reappraisals, including mandatory and voluntary assessments, as required.
* Hold Registered Nurses (RNs) and Enrolled Nurses (ENs) accountable for daily case reporting, care planning, and addressing patient concerns promptly.
* Conduct routine clinical assessments to promptly address identified gaps in care.
About You
* You must be currently registered as a Nurse with AHPRA, with no restrictions.
* Demonstrated senior management experience in the aged care sector.
* In-depth knowledge of the aged care industry, with a keen eye for emerging trends and best practices.
* Proficiency in managing human resource matters, complemented by strong negotiation skills.
* Skilled in utilizing various information technology tools and software, including Word and Excel, with a quick ability to adapt to new skills as required.
