What You Bring
* Current Nurse Practitioner registration with the College of Nurses of Ontario (Extended Class)
* At least 2 years of clinical experience with geriatric and adult chronic disease management
* Strong leadership, organizational, research, evaluation, time management, and interpersonal skills
* Proficiency in MS Office applications
* Knowledge of Point Click Care and RAI-MDS
* Familiarity with the Fixing Long Term Care Homes Act and regulations
* Excellent communication skills
* Punctuality and reliability
