Expert Care and Education Specialist
The Northern Territory Government is seeking a skilled professional to deliver expert care and education in remote NT communities.
This role involves providing specialist clinical outreach care, advice, and education to improve outcomes in early detection and management of preventable chronic conditions.
Key Responsibilities:
* Provide sound professional advice and assessment, planning, review, evaluation, and coordination of comprehensive client care and/or case management.
* Support Public Health Care staff in improving chronic condition management through culturally sensitive approaches.
* Develop and maintain relationships with key stakeholders to enhance service delivery.
* Contribute to the development of policies and procedures related to chronic condition management.
Requirements:
* Current registration with the Nursing and Midwifery Board of Australia or eligible for same.
* High-level clinical knowledge and skills in Preventable Chronic Conditions or related field.
* Strong decision-making and problem-solving skills.
* Commitment to a learning culture through clinical guidance and mentoring.
* Ability to travel to remote communities by 4WD or light aircraft as required.
* Hold a current NT driver's licence (or eligible to obtain) and be confident driving a manual vehicle in regional and remote settings.
