As a Home Care Coordinator, you will work closely with clients, families, and support staff to ensure services are delivered with compassion, professionalism, and efficiency.
Key Responsibilities:
* Manage incoming referrals and conduct client assessments to tailor individualized care plans that meet client goals and needs.
* Coordinate staff schedules and match carers with clients based on their specific requirements.
* Monitor service delivery to maintain quality and client satisfaction through prompt incident response and ongoing feedback.
* Maintain accurate client records and comply with relevant policies and procedures to ensure seamless operations.
We're Looking For:
* Proven experience managing Home Care Packages is essential.
* Current AHPRA Registration.
* Valid Australian Driver's Licence and access to a reliable vehicle.
* Current Police Check.
* NDIS Worker Screening Check.
* Excellent interpersonal skills to build strong relationships with clients, families, and staff.
* Sound understanding of the Aged Care Quality Standards and person-centred care principles.
* Confidence using care management systems and Microsoft Office.
* Strong time management and multitasking ability.
