This position provides case management and assessment within the framework of a multidisciplinary team structure delivering time limited, specific, evidence based, community focused person centred care, with consideration to the specific needs of the target group of consumers referred to the Older People's Community Mental Health Team.
From Helensburgh in the north to North Durras in the south, ISLHD covers an area of 5,687 square kms.
