Where You'll Be Working You will be based at Glen Innes Health Service, delivering services across both community and acute care settings. The role includes regular outreach to surrounding communities, including Tenterfield, supporting improved access to culturally appropriate healthcare across the region.
You will be supported by the Glen Innes Health Service Manager and the Northern Cultural Team Leader (Aboriginal Health), and work as part of the Aboriginal Health Care Coordination Team. This includes regular team huddles and peer support from Aboriginal Health Workers and Aboriginal Health Practitioners across the Hunter New England (HNE) Northern region. Orientation is provided both face-to-face and online to ensure you feel confident and supported in your role.
About the Role This position works across community and inpatient settings to provide culturally safe, holistic care to Aboriginal people and their families. Key responsibilities include:
* Engaging with Aboriginal communities and local services
* Delivering health promotion and education activities
* Providing outreach services to smaller surrounding communities
* Regular outreach clinics in Tenterfield
* Inpatient support within the health service
You will play a vital role in strengthening connections between Aboriginal communities and health services, improving health outcomes through culturally appropriate care and advocacy.
You will also be supported to complete a Certificate IV in Aboriginal Primary Health (Aboriginal Health Practitioner) through TAFE NSW, with ongoing professional development opportunities.
About You You are passionate about improving health outcomes for Aboriginal people and have experience working with Aboriginal communities. To be successful in this role, you will demonstrate:

* Strong communication and engagement skills
* Ability to work independently across outreach and inpatient settings
* Commitment to culturally safe practice and health promotion

Requirements
* Be of Aboriginal or Torres Strait Islander descent (pursuant to Section 14 (d) of the Anti-discrimination Act 1977.
* Be willing to work towards obtaining a Certificate IV Aboriginal Primary Health Care (Practice) within 2 years of enrolment in course and commencement of employment and work towards AHPRA standards to meet Registration as an Aboriginal Health Practitioner. following appointment to the position through supported education, training and study leave.
* Current NSW Drivers Licence and the ability to travel within the local area and throughout Hunter New England NSW LHD (as necessary).
