Support Worker (In-Home Disability Services)
At the Healthscope Independence Services (HIS), we are seeking a passionate and experienced residential/support worker to provide high-quality service to our clients within our Supported Independent Living (SIL) services operating within the NDIS.

* Classification: Level 2 – Pay point will be determined based on experience and qualifications.

About the Role
As a Disability Support Worker, you will be responsible for providing high-quality care and assistance to our clients, helping them achieve their goals and live independently. You will work closely with our clients, supporting them with daily living activities, personal care, and community engagement.
Shift Requirements
* Sleepover following on from a 1500-2300 shift
* Attendant Care 1:1 supports – Mon‐Sun 1000‐1500
A typical day may include:
* Providing personal care support in line with individual support plans
* Medication management
* Assisting participants with daily living activities such as meal preparation and feeding
* Assisting with household tasks such as cleaning, laundry, etc.
* Accompanying and supporting clients in community‐based activities, such as social outings, appointments, and recreational pursuits
* Ensuring all required documentation (including daily notes, communication and progress reports) is thorough and up to date
* Experience with behaviours of concern
* Adhering to all organisational policies, including medication administration, WHS, client rights, privacy, and confidentiality
* Working within a team environment
Qualifications
* Certificate IV in Disability or related qualification
* Minimum of 1 year of experience in disability or healthcare services
* Strong communication, written, interpersonal, and organisational skills
* Flexibility to work across a range of shift types inclusive of weekends
