About the role As our Clinical Care Manager, you’ll play a vital role in ensuring older people receiving Support at Home services experience safe, high‑quality, and truly person‑centred care. You’ll provide strong clinical leadership across care planning, risk management, and service delivery, ensuring every client receives support that aligns with the Aged Care Quality Standards and our organisational values. You will be able to work hours flexible hours and travel around the Broken Bay Diocese. This is a full-time permanent position with the ability to grow this role as the division grows. You will be reimbursed KM's travelled for the role. Duties Lead clinical governance across Support at Home services, ensuring safe, evidence‑based, and high‑quality care that meets all Aged Care Quality Standards. Complete comprehensive clinical assessments and develop personalised care plans that reflect each client’s goals, preferences, cultural needs, and clinical requirements. Oversee and monitor service delivery, client outcomes, and changes in condition—responding promptly to risks, incidents, or clinical deterioration. Provide strong clinical leadership to care coordinators and support workers, offering supervision, mentoring, and training to build capability and confidence. Build trusted relationships with clients and families, supporting informed decision‑making, wellbeing, and culturally safe, person‑centred care. Collaborate with GPs, allied health, hospitals, and internal teams to ensure seamless, holistic care while managing safeguarding, risk, and mandatory reporting obligations. Essential Requirements You need to be a registered nurse with a current AHPRA registration. Minimum 3–5 years’ experience in aged care, community nursing, or chronic disease management. Strong understanding of the Aged Care Quality Standards and the Support at Home (or Home Care Package) service models. Experience in clinical assessment, care planning, and risk management. Knowledge of chronic disease management, dementia care, and palliative care. We strongly encourage First Nations people to apply.
