About the GEM@Home and Rehab@Home Programme

The GEM@Home and Rehab@Home service is a bed substitution model, replicating bed‐based services in the patient's home where appropriate, enabling an earlier supported discharge for SVHM. Patients with conditions associated with rehabilitation needs, ageing, cognitive dysfunction and chronic illness are prioritised for referral. The primary aim of the service is to assist patients in achieving their goals of care in their own home. The service is interdisciplinary and operates 7 days a week with professional support and supervision coordinated by the SVHM Social Work Department.

Position Purpose & Responsibilities

The Social Worker is responsible for the development, implementation and delivery of a high‐quality social work service to patients and families across GEM@Home and Rehab@Home. Working within a flexible, interdisciplinary structure, you will manage a caseload, using clinical skills, evidence‐based practice, effective communication and collaboration.

* Provide optimal clinical interventions: psychosocial assessment, risk assessment, goal setting, intervention, liaison and discharge planning
* Communicate assessment and treatment outcomes to nursing, medical, and allied health staff via documentation and relevant meetings
* Provide discipline‐specific knowledge, skills, consultation and resources
* Supervise staff and students as required
* Participate in quality improvement activities
* Attend and participate in key meetings, supervision, education and training programs

Classifications

SC21 – SC24 ($90,261 – $103,724 per annum, pro rata for part time)

Qualifications

* Bachelor of Social Work Degree / Qualifying Masters Degree
* Eligible for membership of the Australian Association of Social Work (AASW)
* Current Victorian Drivers Licence
* Advanced social work skills, four or more years post‐qualifying professional experience in an inpatient hospital setting
* Demonstrated knowledge of social work theory, practice frameworks and principles in the current healthcare environment
* Experience with NDIS applications and My Aged Care
* Current National criminal history check, or willingness to obtain
* 2 years acute or subacute experience
* High interpersonal skills: excellent communication, collaboration, negotiation and conflict management
* Demonstrated ability to work in interdisciplinary teams at an advanced level
* Well‐developed individual, family work and crisis intervention and care planning skills
* Ability to identify, plan, implement and evaluate new service strategies
* Commitment to staff and/or student supervision
* Commitment to professional development
* Knowledge of quality improvement activities and marketing of the profession
* Knowledge of research/evaluation principles
* Ability to work in accordance with the values of the Ministry of Mary Aikenhead
* Computer literacy skills
* Higher degree or other training relevant to the role (non‐essential)
* Other relevant studies (non‐essential)
