Remuneration : $37.03 – $52.83 per hour

Location : Blacktown Hospital

What You'll Be Doing
Join the TACP team to support older adults transitioning from hospital to home through client-centred rehabilitation and safe discharge planning. Your responsibilities will also include:
* Delivering evidence-based physiotherapy care
* Working closely with a multidisciplinary team
* Supporting clients to achieve functional goals
* Promoting safe and timely community discharge
* Contributing to quality and service improvement
This is a temporary full time position with the possibility of extension, offering an excellent opportunity to further develop your skills in a supportive and collaborative environment.

Essential Criteria
* Qualification: Degree in Physiotherapy OR Equivalent
* Registration: APHRA Registration
Selection Criteria
* Degree in Physiotherapy or equivalent, current registration with the Australian Health Practitioner Registration Agency (AHPRA) and current unrestricted NSW Drivers licence.
* Demonstrated Physiotherapy experience, including aged care and/or rehabilitation and experience delivering Physiotherapy in the community setting.
* Demonstrated computer literacy, high level verbal and written communication and interpersonal skills, including the ability to adapt communication style to suit different audiences.
* Demonstrated high level of organisational and time management skills.
* Demonstrated commitment to professional development, evidence based practice and continuous improvement.
* Demonstrated ability to work both independently and as a member of a multidisciplinary team.
* Demonstrated skill in the comprehensive assessment of older adults, and people with physical disabilities and the development of relevant care plans.
* Demonstrated ability to analyse problems, set priorities and apply a range of options to develop solutions
