About the Position
As a Care Coordinator, you will be the key link between our clients, their families, and our care team. Your primary responsibility will be to ensure that our clients receive high-quality home care services by managing schedules, coordinating care plans, and providing compassionate support.
Main Responsibilities
1. Care Matching: Match clients with caregivers based on their needs and preferences.
2. Schedule Coordination: Schedule and coordinate in-home care services according to client requirements.
3. Communication: Communicate effectively with families, caregivers, and healthcare professionals to ensure seamless service delivery.
4. Care Plan Management: Monitor and update client care plans to reflect changing needs and preferences.
5. Regulatory Compliance: Ensure compliance with NDIS standards and aged care best practices.
6. Customer Service: Provide exceptional customer service and client support.
Requirements
1. A minimum of 2 years' experience in aged care or disability care, preferably with knowledge of NDIS standards and Aged Care Standards.
2. Strong organizational and communication skills, with the ability to multitask in a fast-paced environment.
3. A caring and compassionate attitude, essential for delivering empathetic client support.
4. Proficiency in scheduling software and basic computer skills.
Qualifications
Certificate III/IV in Individual Support (Ageing and Disability) or equivalent qualifications in community services.
