About the Opportunity
A Home Support Team Member (HSTM) plays a vital role in helping older adults live safely and independently at home while maintaining their dignity and lifestyle.
Working one‑on‑one with clients, you will provide personalised care and support, building trusted relationships and delivering services that make a real difference in everyday life.
Key responsibilities
* Provide one‑on‑one support including personal care, meal preparation and household assistance
* Assist with daily living tasks and provide social support and companionship
* Support client health and wellbeing through medication reminders and coordination of medical appointments and transport
* Observe, monitor and accurately record changes in physical health, cognition or social engagement
* Report concerns promptly through appropriate reporting processes
* Build respectful, trusting relationships that support choice, independence and client control

About You
As a caring and reliable person who is passionate about supporting older people to live safely and independently at home, you will,
* Build positive relationships and communicate in a friendly, patient and respectful way
* Treat every client with dignity, kindness and understanding
* Notice changes in a client’s wellbeing and speak up when something doesn’t seem right
* Understand both the practical and emotional needs of older people
* Be organised, dependable and manage your time well, always following workplace health and safety guidelines
Essential Criteria
* Certificate III in Individual Support / Aged Care or equivalent
* First Aid, CPR and Manual Handling
* Full Australian Driver’s licence
* Registered & roadworthy vehicle
* National Police Check within 12 months validity.
