Responsibilities
* Ensure patients who self-identify as Aboriginal or Torres Strait Islander have their psychosocial needs assessed and are supported and connected appropriately with relevant services.
* Collaboration with multidisciplinary healthcare teams to provide culturally sensitive, safe and accessible health care to patients in the North West region of Tasmania.
* Assist vulnerable people experiencing an acute medical event and support them with lifestyle adjustments.
* Participate in inpatient discharge planning.
* Identify and address social care needs during and post hospitalisation.
* Access cultural support, professional development and supervision from a senior social worker.
Qualifications (Essential)
* Identify as Aboriginal and possess sound knowledge of health issues impacting Aboriginal and Torres Strait Islander communities.
* Hold a tertiary qualification or program of study approved by the Australian Association of Social Workers.
* Eligible for membership with the Australian Association of Social Workers (AASW).
* Overseas qualified social work candidates must provide a "certification of eligibility" letter from the AASW confirming eligibility for membership.
* Hold a current Working with Children Registration.
Desired Qualifications
* Experience working with or in Aboriginal communities.
* Current driver's licence.
Salary and Packaging
Annual salary range: $73,834 to $111,364 per annum, plus a 12% superannuation contribution.
Salary packaging includes benefits such as living expenses up to $9,010 per FBT year, meal entertainment up to $2,650 per FBT year, and other eligible benefits.
Professional Development Allowance: up to $1,000 per annum. Additional allowances may be available subject to qualifications and the personal upgrade scheme.
