Nepean Blue Mountains Local Health District (NBMLHD) is a wonderful place to expand your career and grow your skills and knowledge. As a recognised leader in the healthcare industry, we provide a range of public health services to the Nepean, Blue Mountains, and Lithgow Region.
The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people's homes, other community venues as well as virtual care (telehealth).
A Permanent Full time opportunity is now available for a Social Worker to join the Community Supportive Care Team located at our Lemongrove Campus.
The primary purpose of the role within the interdisciplinary Community Supportive Care team is to work with other community health teams, general practitioners, hospital teams and specialist palliative care services to co-ordinate and deliver best supportive care to clients with late stage degenerative and chronic conditions within the last 2 years of life.
The goal of the Community Supportive Care team is to promote and support a client's function and independence at home, and to provide high level psychosocial support for their family and carers.
Your role as the Senior Social Worker will provide a superior level of competent social work interventions providing care coordination to clients of the Community Supportive Care service, which will include holistic, and person centred comprehensive psychosocial assessments in the client's home environment.
The Senior Social Worker will develop measurable and realistic goals, care plans and priorities for timely intervention and implement and review the care plan with the team, client and/or carer and will provide education and information to clients, carers and staff regarding ongoing intervention and community resources and ensure close liaison, communication and timely referrals to other team members and service providers.
As the Senior Social Worker, you will demonstrate advanced reasoning skills and will operate autonomously with minimum direct clinical supervision providing care to the clients of the Community Supportive Care service and will prioritise and manage their individual caseload to ensure timely access to services.

**What you will bring to the role**
- Degree in Social Work and eligibility for membership of the Australian Association of Social Workers. Overseas qualifications must supply the AASW Overseas Assessment number.
- Demonstrated high level of clinical expertise and knowledge in the assessment and management of clients with chronic and complex conditions including experience in palliative care across community or hospital settings.
- Demonstrated leadership ability to provide clinical support, education, and consultation to health professionals in area of specialty to improve client outcomes and service design.
- Demonstrated ability to communicate effectively with clients, carers and stakeholders and provide advice to managers on service development.
- Demonstrated commitment to evidence-based practice and quality improvement.
- Ability to work independently with mínimal supervision and maintaining involvement within the wider interdisciplinary team.
- Current Driver's Licence and willingness to use for work purpose travel. This may include driving long distances across the LHD.
