The Role
We are seeking a Support Coordinator to support people with psychosocial disability to participate more fully in the community and enhance their capacity for social and economic participation.
This includes:
* Strengthening their ability to coordinate and implement supports in their National Disability Insurance Scheme individualised funding packages
* Supporting their engagement with mainstream, formal and informal services
* Developing capacity and resilience in their network
Key Responsibilities
As a Support Coordinator you will:
* Develop a relationship with people including their family and carers
* Support a person to develop and implement their individual recovery plan
* Coach a person to increase recovery skills and personal capacity, including motivation, strengths, resilience, and decision-making
* Collaborate with broader systems including available services and resources
* Support people to become active participants in the NDIS
Requirements
To be successful in this role, you will need:
* A minimum of a Cert IV in a Community services related discipline (ideally a completed degree or Diploma)
* Experience in a human services environment, particularly experience in understanding service delivery and how it can best deliver for clients with persistent and complex mental health
* Experience in assisting participants to test their eligibility of the National Disability Insurance Scheme or transition them to other appropriate supports
* Highly-developed communication, interpersonal and negotiation skills including the ability to work and communicate effectively with clients, their families, community agencies and other professionals
* Ability to meet deadlines in a high demand work environment and to exercise judgment and initiative
* Excellent organisational and time management skills
* Must be eligible to apply/currently hold a Working With Children Check
* Current Driver's License
