Connect4U Australia is a NDIS-registered, independent provider of Level 2 and Level 3 support coordination and psychosocial recovery coaching. We are seeking enthusiastic, outcome-focused Support Coordinator / Psychosocial Recovery Coach (contractor positions) to join our team – Capalaba and the surrounding area.
Qualifications
Support Coordinator / Recovery Coaching experience or human services case management experience is essential. Qualifications and experience with psycho-social disability are essential. Lived experience is also preferable. The successful applicants must live in the relevant geographical area and have good local knowledge and connections.
Support Coordinator Responsibilities
- Implement their plan and increase their capacity to manage/direct their own supports.
- Have greater opportunities to explore and connect with their community and alternative support options.
- Strengthen their informal support networks.
- Maximise the value for money they receive from their supports.
- Access and use the NDIS Participant Portal.
- Effectively communicate and maintain relationships with the NDIA.
Psychosocial Recovery Coach Responsibilities
- Develops recovery-enabled relationships, based on hope.
- Supports NDIS participants with recovery planning.
- Coaches NDIS participants to increase recovery skills and personal capacity, including motivation, strengths, resilience and decision-making.
- Supports NDIS participants to engage with the NDIS.
The successful applicants will have a caseload of Participants who have either support coordination or psychosocial recovery coaching funding. The position offers work‑life balance with work‑from‑home and flexible schedule, to be completed on a full‑time equivalent basis. Support Coordinators / Psychosocial Recovery Coaches are mentored by dedicated Managers who are experienced practitioners in the support coordination field.
