Yallambee has exciting opportunities for experienced and enthusiastic Personal Care Workers to join our residential care services team.

Our current Senior Clinical team includes a Clinical Operations Manager, Clinical Care Coordinators, Quality Manager and Nursing Team Leaders to support you in your role as a PCW.

As a Personal Care Worker, you will play a vital role in providing assistance and support to individuals requiring personalized care.

You will work closely with consumers to help them with daily activities, promote their independence, and ensure their comfort and well-being.

Your compassionate and empathetic nature will be crucial in building rapport with consumers, fostering trust, and enhancing their overall quality of life.

The successful applicants will be part of a dynamic team of committed, dedicated and caring staff who understand each resident’s needs, goals and preferences to optimise their health and well-being of each individual.

Personal Care Workers are an integral member of the care team, delivering personal care to our frail and aged residents.

The Personal Care Worker works under the direction of the Registered Nurse or Enrolled Nurse and is responsible to ensure their focus is primarily on delivering the highest quality of care and customer service, within their scope of practice, for each individual resident.

In addition, we have been awarded a 5 Star Rating in Staffing and a 5 Star Rating in Compliance.

Key Performance Areas include:

- Ensures the highest standards of practice and care delivery.

- Compliance with the Aged Care Standards, Yallambee’s Policies and Procedures and Yallambee’s Vision, Mission and Values Policy.

- Delivery of personal care in accordance with assessed need, within their scope of practice and as directed by the Registered Nurse and Enrolled Nurse
- Maintains and enhances resident’s health and well-being, capabilities, independence, choice, privacy, dignity and safety.

- Ensures a safe working and living environment.

- Highest standard of documentation including, charting, progress notes, schedules, duty lists.

- Incident identification, reporting and documentation.

- Communicating professionally and appropriately to residents, families and referring questions and concerns to Nursing staff.

- Participation in auditing and continuous quality improvement activities
- Excellent communication and customer service skills

Key Selection Criteria

- Excellent interpersonal and communication skills
- Certificate III or IV in Aged Care / Individual Support, completion of the subject assist with medication highly desirable.

- Demonstrated ability to work as a member of a multidisciplinary team.

- Must have a person-centered approach to care and service delivery.

- Previous experience in Aged Care is highly desirable.

- Knowledge of Aged Care Standards

Appointments

All appointments are made subject to the candidate providing a valid NDIS Workers Screening Check, evidence of 2025 influenza vaccination and evidence of COVID-19 vaccination.
