Your Role

As an AOD Care Navigator, you will support individuals through their recovery journey by providing case management and care coordination. Working closely with local services and a multidisciplinary team, you will deliver person‐centred support that builds resilience and improves access to treatment and community services. The New England AOD Hub is a exciting initiative funded by NSW Health, delivering specialist alcohol and other drug support across the region. The service works in partnership with Armajun Aboriginal Health Service and other local providers to support people experiencing complex needs, including mental health challenges, risk of homelessness, and involvement with the justice system.

Your Responsibility

* Provide case management and care coordination
* Conduct intake, assessments and ongoing client support
* Deliver AOD programs and psychoeducation
* Develop and implement care plans
* Work collaboratively with local services
* Support families and significant others
* Maintain accurate documentation and records

You Will Need

* Certificate IV in Community Services or related field
* Experience working with vulnerable communities
* Knowledge of AOD and local support services
* Strong communication and organisational skills
* Ability to respond to complex and crisis situations
