* Part-time role: 24 hours per week (16 hours in Community Rehabilitation Program and 8 hours in ABI Community Rehabilitation Service)
* 6 months contract

The Department
Community Rehabilitation Program (CRP)
* Provides home and centre-based rehabilitation as well as Falls Clinic services.
* Focus on goal-directed, community-based rehabilitation following injury or hospitalisation.
* Multidisciplinary team includes physiotherapists, occupational therapists, speech pathologists, dietitians, neuropsychologists, clinical psychologists, allied health assistants, rehabilitation medicine doctors, pharmacists, registered nurses, and social workers.
Acquired Brain Injury Community Rehabilitation Service (ABI Comm Rehab Service)
* Provides statewide specialist rehabilitation for people with moderate to severe ABI, including traumatic brain injuries, stroke, hypoxic brain injuries, and other non-progressive brain injuries.
* Offers long-term management and support, focusing on evolving needs.
The Role
CRP
* Provide high-quality clinical nutrition and dietetic services to referred clients.
* Conduct treatment programs in clients' homes or at Caulfield Hospital.
* Manage varied caseloads including neurological, orthopaedic, cardiac, spinal, pulmonary, and functional decline diagnoses.
* Deliver flexible, client-centered rehabilitation services.
ABI Comm Rehab Service
* Counsel patients on nutrition and healthy eating habits.
* Develop and evaluate meal plans considering cost and preferences.
* Document client progress through reports.
Qualifications and Experience
* Bachelor in Nutrition and Dietetics or equivalent.
* Accredited Practicing Dietitian (APD) status.
* Registration with AHPRA.
* Victoria Driver's License.
* Membership of the Dietitian Association of Australia (DAA).
* Working with Children Check and Criminal History Check.
* Registration as an NDIS Provider.
* NDIS Worker screening check and online training.
* Experience or knowledge in home enteral nutrition (essential if providing such therapy).
Desirable Skills and Experience
* Experience working with people with disabilities.
* Minimum 3 years' experience.
* Rehabilitation or aged care experience.
* Ability to work effectively in an interdisciplinary team.
* Understanding of continuous quality improvement principles.
* Evidence-based service evaluation and modification skills.
* Strong communication and interpersonal skills.
* Knowledge in orthopaedics, neurology, and gerontology.
* Participation in relevant professional development.
Highly Desirable
* Recent community-based rehabilitation experience.
* Experience in a Grade 2 Dietitian role in a public health service.
