Your Role as our Youth Connect Worker
You'll support our team to enable opportunities and resources to young people to maximise their capacity to be independent, self-reliant and connected to appropriate social and community resources. This includes the development and facilitation of individual and group youth activities to advocate, mentor and guide our young people.
You'll deliver person-centred and quality care coaching and mentorship by:
* Engaging young people to provide them with direct practical support in coordination with case workers.
* Developing and maintaining professional communication and relationships with young people and relevant stakeholders.
* Providing targeted outreach/home visits to young people
* Responding to crisis situations, implementing appropriate intervention strategies
* Ensuring high quality, up to date, accurate case notes/materials and maintenance of accurate data and updates in systems.
* Providing timely follow up support and where required transport young people to appointments where required and appropriate.
This is a part-time position consisting of 24 hours per week.
What We're Looking For
* Completed tertiary qualification in youth work or relevant human services field.
* Comprehensive understanding of issues and service delivery relevant to working with young people.
* Ability to function as part of a team, autonomously and with minimal supervision.
Essential Requirements
* Proof of COVID-19 vaccination status (or as required by legislation and WMQ policy)
* Relevant probity checks required by legislation and WMQ Limited policy.
* Current Driver's Licence and willingness to travel for work
