Job Description
Key Responsibilities
You will work with individuals and families providing assessment, counselling, advocacy, referrals and community education, while collaborating closely with multidisciplinary teams.
* Provide Social work assessment, intervention and evaluation to allocated clients.
* Provide practical and emotional support to clients and their families, in collaboration with and through team members as required.
* Manage a caseload effectively and maintain accurate records.
* Link clients to community resources and internal services.
* Advocate on behalf of clients to improve outcomes.
* Support clients to understand and adjust to vision loss and diagnosed eye conditions.
* Contribute to community education and awareness activities.
* Supervise social work students (where eligible).
* Maintain client records in the client management system.
* Perform other duties as reasonably required.
Desired Skills and Experience
About you
You are a compassionate and capable social work professional who thrives in a client‐centred environment and values meaningful work.
* Tertiary qualification in Social Work
* Eligibility for full membership of the Australian Association of Social Workers (AASW)
* Understanding of the psychosocial impacts of disability and vision loss
* Strong written and verbal communication skills
* Ability to apply core social work practice methods
* Commitment to inclusion and person‐centred practice
* Alignment with EverAbility Group values
* Experience in disability, aged care or community services
* Knowledge of community resources and service systems
* Familiarity with relevant legislation and frameworks
* Research or program development experience
Required Compliance Documents
* National Police Clearance
* Registration: Full membership of the Australian Association of Social Workers
* Certificates: Tertiary qualification in Social Work
