**Main Duties and Responsibilities**:
Assist older people living independently in the community with activities relating to personal wellbeing, household management, and lifestyle,including:
- personal hygiene, grooming and presentation
- general household, cleaning and environmental maintenance tasks
- activities of interest in the home and local community
- activities related to the maintenance or improvement of health and wellbeing o other reasonable requests made by the client
- Deliver services that respect the rights of the client, including maintaining confidentiality; and respecting the individual values, customs, preferences and spiritual beliefs of clients and their families/carers
- Maintain an awareness of the client's current circumstances and needs, reinforce their capacities, and encourage each client's independence
- Encourage and support clients to maintain their interests, friendships and social interactions
- Participate in the implementation of client activity plans or rehabilitation plans, including maintaining accurate and concise records and reports
- Ensure ongoing communication with Manager/CEO about the health, wellbeing and progress of assigned clients, and advise any changes in the client's circumstances or needs
- Implement the organisation's internal controls, information management systems, processes and procedures
- Maintain and enhance skills through participation in external and internal education and developmental activities as directed
- Provide high-quality customer service to all external and internal customersIdentify and communicate opportunities for service improvement within the organisation
- Ensure all behaviour, communications and documentation of self and direct reports are consistent with relevant legislation; the organisation Vision, Mission and Values; and its policies and procedures, including the Code of Conduct, Philosophy of Care, Privacy and Confidentiality, Work Health Safety, Quality Management, Bullying and Harassment, etc.
- Undertake projects and other duties commensurate with skills, knowledge and experience.
**Qualifications and Requirements**:
- An understanding of the ageing process and health issues relevant to older people and people with a disability
- Certificate III in Individual Support (Ageing, Home and Community or Disability)
- Cert IV in Ageing or Disability Support (desirable)
- Knowledge of the local geographical area
- Commitment to ongoing professional development
- Current driver's licence
- Current police check
Desirable:
- Additional education / qualification in dementia care
- Experience in medication administration
- Ability to speak in another language in addition to English.
**Skills and Experience**:
Demonstrated ability to prioritise work, manage workload and cope with change in work demands.
- Experience in supporting older people living in their own homes.
- Demonstrated ability to communicate effectively with people with older people, their families/carers, staff and other health professionals.
- Ability to work with clients from a range of social, financial and cultural backgrounds.
- Ability and willingness to undertake a wide range of tasks including assisting clients with personal hygiene, grooming and presentation
- Ability to complete written notes and forms clearly and accurately.

**Salary**: $21.87 - $38.62 per hour
