We're looking for a dedicated Case Manager to join our Specialist Homelessness Services team.

Job Description
The primary focus of this role is to deliver high-quality case management services to clients with complex needs.
This involves conducting comprehensive assessments and developing individualised support plans, as well as advocating for clients and coordinating referrals to housing, health, legal, and social support services.
Maintaining accurate and timely documentation is also crucial in this position.

* Casework: Deliver casework services to clients, including assessing needs and developing individualised support plans.
* Advocacy: Advocate for clients and coordinate referrals to relevant services.
* Documentation: Maintain accurate and timely documentation of client progress and service delivery.

Requirements
To be successful in this role, you'll need the following skills and qualifications:
* Tertiary qualification in a social science or related field.
* Casework experience in an associated field.
* Understanding of, and demonstrated ability to work with Trauma-Informed Care principles.
* Current Working with Children Check (NSW).
* Satisfactory Criminal Record Check.
