About The Role
As the Dietitian for TACP, you will provide independent, evidence-based nutrition assessment, management, and care coordination for older adults and people with physical disabilities. This is a newly established service within the program, making you an integral and highly valued member of the team.
Your role includes contributing to data collection, outcome measurement, consumer feedback processes and quality improvement initiatives to ensure continuous enhancement of the TACP Dietetics service.
A typical day involves travelling across the region to conduct home-based nutrition assessments, liaising with the multidisciplinary team to support client progress, and ensuring each client receives safe, personalised and practical nutrition care that supports their rehabilitation goals.
This role is ideal for a clinician who enjoys autonomy, collaborative team practice, and the opportunity to build and shape a new dietetic service within an established aged care program.
About You
You are a motivated, skilled and compassionate Dietitian with a strong interest in supporting older adults during periods of transition and recovery. You thrive in community-based work, feel confident working independently, and value the shared expertise of a multidisciplinary team.
You bring advanced skills in the comprehensive nutrition assessment of older people, strong clinical reasoning, excellent communication, and the ability to build supportive relationships with clients, carers and colleagues. You're committed to evidence-based practice, goal-oriented care, and improving outcomes for clients with diverse and complex needs.
Why Work for Us?
In this role, you will be well supported through face-to-face training and a thorough orientation to help you transition confidently into the service. You'll work alongside Registered Nurses, Enrolled Nurses, Physiotherapists, Occupational Therapists, Speech Therapists, Domestic Support, the TACP Coordinator, NUM SAS & TACP, the Aged Care Service Manager, Psychogeriatric services, Allied Health teams and dedicated administrative staff.
At Hunter New England Health, we are committed to continuous learning. You'll have access to ongoing professional development, supervision, clinical guidance, and opportunities to build specialist skills in aged care and community rehabilitation.
This is your chance to make a meaningful difference to clients at a pivotal point in their recovery and independence—while enjoying the work-life balance, support and culture that HNE Health is known for.
