Assess, support and advocate for renal and cardiothoracic patients, delivering expert psychosocial care within a fast-paced acute hospital environment.

Remuneration: $38.51 - $54.95 per hour

What you'll be doing
In this Social Worker role, you will provide high-quality psychosocial care to patients and families navigating acute and chronic illness, while contributing to service development and multidisciplinary care planning. You will:

* Conduct comprehensive psychosocial assessments and provide expert clinical input
* Deliver counselling, crisis intervention, advocacy and discharge planning support
* Collaborate with medical, nursing and allied health teams on complex care matters
* Participate in quality improvement, education and evidence-based practice activities
* Maintain accurate documentation and contribute to service reporting and outcomes

Essential Criteria:

* Qualification: Degree in Social Work with eligibility for membership of the Australian Association of Social Work (AASW).

Selection Criteria:

* Demonstrated understanding of psychosocial issues associated with acute and chronic illness.
* Demonstrated capacity to provide counselling and case management with patients/families with illness adjustment, end of life care and with complex discharge issues
* Demonstrated clinical experience or understanding of the psychosocial impact of grief, loss and bereavement.
* Demonstrated understanding of issues associated with working with Culturally and Linguistically Diverse client group.
* Demonstrated well-developed time management skills with excellent written and verbal communication, including report writing skills and high-level computer literacy.
* Demonstrated ability to work with minimal supervision and as part of a multidisciplinary team including the ability to interact constructively and collaboratively with a diverse range of stakeholders, throughout all organisational levels
* Knowledge of quality assurance and / or clinical practice improvement activities
* Unrestricted drivers licence and availability to participate in the After-Hours Crisis On Call Social Work Service.
