Our organization seeks a dedicated and compassionate Community Registered Nurse to join our Home Care team. The successful candidate will provide high-quality nursing care to clients in their homes, focusing on supporting older adults to maintain their independence and dignity.
Key Responsibilities
* In-home clinical assessments, reviews, and care planning for clients in the East and Outer Eastern suburbs
* Provide expert clinical advice and support to clients, facilitating referrals to specialized services where necessary
* Collaborate with local allied health professionals to ensure responsive care in clients' homes
* Lead by example, promoting a harmonious team approach through effective coordination of daily routines and adherence to safe practice measures in medication management
Requirements
* Bachelor of Nursing or equivalent qualification with current AHPRA Div 1 Nursing registration (required)
* Minimum 3-5 years experience as a Registered Nurse in Community, Commonwealth Home Support, or District Nursing (required)
* Team player with excellent communication skills, flexible in approach, and positive outlook
* Endorsed for Continence Assessments and Prescription (highly desirable)
