Job Title: Rehabilitation Consultant
We are seeking a skilled Social Worker to join our multidisciplinary team of allied health professionals as a Rehabilitation Consultant.
As a key member of our team, you will play a vital role in helping individuals regain their independence and achieve their goals in life and work.
Key Responsibilities:
* Undertake assessments relevant to your discipline to develop and implement effective rehabilitation plans.
* Design and implement intervention programmes using occupation to improve functional performance, independence and work capacity.
* Identify barriers and design, implement and monitor programs that support people to achieve maximal work participation.
* Understand various healthcare and rehabilitation funding schemes and empower clients to achieve their wellness and recovery goals.
* Liaise with clients, family members, funding bodies, medical practitioners, employers and stakeholders to achieve successful outcomes.
Requirements:
* Australian recognised degree qualification in Social Work (AASW).
* Ability to engage clients and stakeholders in the wellness and work journey.
* Commitment to a high level of customer service to clients and customers.
* Willingness to work in the community (must have driver's license and access to own vehicle).
* Current or able to get a Working with Children Check and Police Clearance.
