About Allied Health at Cabrini

The Role
We are currently seeking a passionate and dedicated Grade 1 Social Worker to join our acute care team in a busy, large private hospital.
As a Social Worker at Cabrini, you will:
Deliver high-quality patient care using sound clinical judgment and communication
Work collaboratively in a multidisciplinary team, including doctors, nurses, physiotherapists, occupational therapists, dietitians, speech pathologists, and external service providers
Conduct comprehensive psychosocial assessments
Develop and implement targeted intervention plans in partnership with patients, families, and carers
Contribute to safe and effective discharge planning and community referrals
What We’re Looking For
Strong communication, computer literacy, and documentation skills
Eligibility for membership with the Australian Association of Social Workers (AASW) – essential
Capacity to work within a performance-review and feedback framework – essential
Previous experience in a hospital setting – desirable
Solid knowledge and application of evidence-based practices in social work, including in mental health settings
A commitment to ongoing professional development and participation in quality improvement or research projects
