Join Country Health Connect in Angaston as a Clinical Nurse - Cardiac Rehabilitation and play a key role in delivering integrated, community-based cardiac services. You'll work within a values-driven, supportive team focused on improving access, continuity of care, and patient outcomes across the region.
This is a rewarding opportunity for an experienced cardiac nurse to contribute to service development, provide clinical leadership, and make a meaningful difference for clients living with cardiac conditions in regional South Australia.

WHO ARE WE LOOKING FOR?
* Are you an experienced Registered Nurse with a strong background in cardiac nursing?
* Do you enjoy working autonomously while collaborating across multidisciplinary and regional services?
* Are you committed to improving access to care and supporting clients to manage cardiac conditions closer to home?
HOW WILL YOU CONTRIBUTE?
* Deliver evidence-based cardiac nursing care through Country Health Connect in Angaston.
* Support cardiac rehabilitation services to reduce hospital presentations and improve client outcomes.
* Take accountability for your practice and provide guidance to less experienced staff as required.
* Assist with waitlisted clients, including supporting clients from Gawler and surrounding areas when requested.
