The annual salary for this position is $76,515 to $77,500 plus superannuation.

Duties
* Assessing client needs and planning, coordinating, and delivering educational, training, and support programs.
* Conducting client assessments and interviews to determine the nature and extent of their challenges.
* Monitoring client progress and preparing detailed progress and outcome reports.
* Referring clients to appropriate agencies and services for additional support.
* Assessing community needs and identifying resources related to health, housing, employment, education, and social services.
* Working in partnership with community groups, welfare organisations, government bodies, and private sector stakeholders to address local issues and promote available services.
* Supporting families and providing care, guidance, and education for children and individuals with disabilities in residential or institutional environments.
* Overseeing and supporting individuals under probation or parole supervision.
* Assisting young people in managing social, emotional, and financial challenges.
* Preparing funding proposals, resource plans, and reports for government agencies and other key stakeholders.
Qualifications
* Must possess a Diploma of Community Services.
* Should have more than one (1) year of relevant experience.
Residential Care Officer (Aged Care)
About the Role: As a Residential Care Officer, you will play a vital role in supporting older adults to live with dignity, comfort and choice. You'll deliver compassionate, person-centred care in a welcoming community, working alongside a dedicated team that truly values respect, inclusion and quality of life.
Duties
* Keep resident documentation accurate and current, including daily notes, care assessments, and care plans.
* Monitor and record any changes in residents' needs, promptly reporting concerns.
* Make residents' beds and ensure fresh, clean linen is always available.
* Keep residents' clothing stored neatly and organised.
* Correctly return laundered clothing to the appropriate residents' rooms.
* Assist with meal service, supporting residents during mealtimes as needed.
* Help deliver specialised nursing care under supervision, following individual care plans.
* Provide consistently high standards of care and support to all residents.
* Encourage residents' independence by assisting with personal care respectfully and in line with their care plan.
* Communicate effectively with colleagues, residents, families, GPs, and allied health professionals to ensure prompt, courteous service.
* Uphold residents' rights to privacy, confidentiality, choice, and decision-making always.
* Offer emotional support and companionship to residents.
* Engage proactively and cooperatively as part of the wider care team.
* Build and nurture positive working relationships with colleagues and stakeholders.
