We're on the lookout for a remarkable individual brimming with enthusiasm and passion what they do. The role of a Participant Care Partner is pivotal in ensuring the delivery of comprehensive, participant-centered care, managing care plans, and coordinating services within the community.
Key Responsibilities:
* Enablement & Re-ablement: Support your participants to maintain or regain skills, Embed therapy recommendations from Occupational Therapists, Physiotherapists or Nurses into daily routines. Encourage gradual skill-building, confidence and autonomy. Promote dignity-of-risk while maintaining safe practices.
* Social, Emotional and Community Participation: Offer companionship that supports wellbeing and reduces isolation. Encourage engagement in hobbies, community activities and routines.
* Observation, Documentation & Reporting: Observe/note changes in mobility, cognition, mood, appetite, or safety. Document service delivery and observations in organizational systems. Report risks, incidents, or deterioration to coordinators, RN or Allied Health. Respect confidentiality and privacy at all times.
* Support Goals and Care Plans: Work with your participants to understand their priorities and preferences. Follow the established support plan, risk plan and therapy goals. Provide feedback to enable care plan adjustments when needs evolve. Encourage independence and decision-making aligned with your participant's own goals.

Mandatory Requirements:
* 2 years experience in case management within aged care services.
* Australian Driver's License.
* Valid police check from within the last 3 months.
