Amplar Health Home Hospital (AHHH) is expanding its multidisciplinary team and seeking an experienced Clinical Pharmacist with strong hospital experience to help design, lead, and deliver high‐quality medication management across all our programs.

This role is ideal for an advanced hospital pharmacist who thrives in a collaborative, fast‐evolving clinical environment and is passionate about advancing the quality use of medicines. Join our team on a full‐time basis to:
* Deliver expert clinical pharmacy services, ensuring safe and effective medication management.
* Provide evidence‐based therapeutic advice to patients, carers, and clinicians.
* Apply independent professional judgment in medication review, reconciliation, and optimisation.
* Support continuity of care across transitions, including admission, discharge, and handover processes.
* Contribute to medication governance, policy development, and service improvement initiatives.
* Participate in research, quality improvement, and evidence‐based practice activities.
* Engage in consumer education to promote safe and informed medication use.
* Ensure compliance with all standards and organisational policies.
About you
* Registered Pharmacist with current unrestricted AHPRA registration.
* Experience in hospital pharmacy or sub‐acute setting.
* Detailed orientated with a high attention to detail.
* Strong clinical knowledge and ability to apply evidence‐based practice.
* Excellent communication and teamwork skills.
* Commitment to patient safety, quality improvement, and continuous learning.
* Hold a South Australian Working with Children Check and National Police Clearance (or are willing and eligible to obtain).
* If successful, willing and able to provide proof of up‐to‐date vaccinations.
