An exciting opportunity for an experienced Clinical Nurse Consultant to join our innovative Virtual Care Service. This role supports coordinated, patient-centred palliative care through expert advice, care planning, and pathway development.

**What you'll be doing**
This pivotal role involves providing comprehensive care planning and coordination for patients and carers, liaising with both community and in-patient palliative services. This position is to offer expert clinical advice to patients, carers, and healthcare professionals, develop and implement effective care plans, and organise referrals while collating clinical information. Additionally, this role is to develop clinical pathways to optimise service delivery and innovate best practice models of care tailored to the patient journey. Working collaboratively with the Supportive and Palliative Care Network, the incumbent will act as a change agent to promote practice development and quality initiatives, ultimately enhancing the patient, staff, and referrer experience.

- Current registration with the Nursing and Midwifery Board of Australia as a Registered Nurse (Division 1) who has a minimum of 5 years full time post graduate experience, with at least 3 years full time equivalent experience in Palliative Care Nursing. In addition, the applicant must have an approved post graduate qualification (minimum Grad Diploma/Certificate level) or evidence of working towards Masters level qualification, relevant to Palliative Care Nursing.
- Demonstrated clinical leadership skills and an ability to work collaboratively within a multidisciplinary framework with a demonstrated commitment to excellence in nursing practice.
- Demonstrated ability to communicate effectively and contribute nursing expertise to the improvement of care within a multidisciplinary care team.
- Demonstrated experience utilising knowledge and advanced nursing skills in the assessment, care planning and provision of safe clinical care for patients with needs related to Palliative Care.
- Demonstrated commitment to quality improvement through the use of evidence to evaluate care and implement change. Demonstrated advanced ability to assess, plan, initiate and manage complex cases.
