Position Objectives:
It is the duty if the Disability Support workers to implementing training and support services aimed at maximising the living, employment, social and recreational skills of people with a disability, community inclusion, independence, decision making and personal choice should be promoted through these supports, with a focus on individual needs.

This position exists to support individuals to achieve their personal ambitions and to empower people to have greater control and authority in their lives.
In supporting people to achieve their aspirations and to be included in their communities your work will involve a broad range of aspects including partnering with people and their families, referral, relationship and communication facilitation, skill development, partnership with organisations to support inclusion and increasing the capacity of community venues through training and education.
In your work there will be a continued emphasis on sustainability and safeguarding of individuals achievements and lifestyles.
Primary Responsibilities
Support Workers are responsible for assisting in the delivery of quality programs for Neway Disability Services participants in accordance with the identified requirements of the PCP
The Individual Lifestyle Arrangements program provides high quality community-based supports to people of all ages with a disability. The program aims to enrich people’s quality of life through a Person-Centred Active Support approach which promotes people’s:
o Dignity and respect in all aspects of their lives.
o Skills that are developed and utilised in a way which achieves their identified goals
o Participation in community activities.
o Choice in all aspects of decision-making which affects them.
o Relationship building with other people.
Duties
· Facilitate and encourage participation through the use of effective and augmented communication and interaction strategies.
· Participate in Staff induction
· Coordinate with participants, families and carers as required.
· Assist Participants to develop and monitor an individual plan that reflects their needs based on the fundamentals of Person Centred/Essential Lifestyle Planning
· Take part in Staff orientation
· Undertake all rostered or other duties as directed in a manner ensuring satisfactory service delivery according to the Disability Services Standard and Organisational Policies and Procedures
· Actively participate in program planning, training sessions and team building conducted by Management
· Implement and monitor Behaviour Management Strategies as required.
· Exercise knowledge/skills to assist in the co-ordination and delivery of programs in a manner that ensures best practice processes are followed.
· Provide support and assistance in the provision of personal care to all Participants as required.
· Actively participate in annual performance appraisals conducted by Management.
