About The Role

We are looking for Youth Lead Practitioners to support adolescents and young adults aged 9-25 as they navigate life with a disability, from adolescence through to early adulthood.

Responsibilities

* Lead the interdisciplinary youth team that plans a young person's NDIS funded therapy package, acting as the main contact for adolescent and young adult participants and their families
* Utilise assessment and intervention skills to support youth participants to achieve their educational, personal, social, and working life goals
* Work within the social model which means you'll have worked and delivered therapy in diverse settings, from homes and schools to playgrounds, workplaces and community centres
* Held meaningful, future-focused conversations with participants and their supports
* Empower participants and their families to develop habits and routines
* Build meaningful relationships with your participants and their supports (family, carers, friends)
* Document and billed for each session through our online participant management system
* Attend the weekly team meeting in the Hub, sharing stories of impact, coffee and laughs
* You'll have met with your Team Leader and Therapy Mentor to tap into their clinical knowledge and/or support

Who We Are Looking For

To work as a Youth Lead Practitioner, you will have a relevant tertiary qualification and registration.

* If you are a Youth Worker, you will have a Bachelor of Master's degree in Youth Work or Community Services and YWA membership
* If you are a Social Worker, you will have a Bachelor or Master's degree in Social Work and AASW membership
* If you are a Specialist Educator, you will have a Bachelor or Master's degree in Special Education and AASE membership
* If you are a Registered Nurse, you will have a Bachelor of Nursing and AHPRA registration.

You Will Also Need

* Minimum 12 months experience working with youth aged 9-25
* A positive and flexible approach to work
* Strong communication skills and be a team player
* Commitment to personal and professional growth
