We are looking for a number of Support Coordinators to join our team - both experienced and aspiring

We embrace flexibility and work/life balance When we aren't visiting participants, we work flexibly from home and the Hively Coordination office in East Brisbane.
We understand living with a disability comes with inherent challenges and we provide excellent training and support for our team to fulfil this role and to ensure our participants have the best possible outcomes under the NDIS framework.
This is a Brisbane based position, where you will be required to work in our East Brisbane office, from home and in the wider Brisbane community in the following regions depending on where your participants are located:
* Logan
* Redland
* Ipswich
* Moreton Bay
* Gold Coast

WHAT YOU CAN EXPECT FROM THE ROLE
You will partner with participants and their families to provide person-centred Support Coordination services for NDIS participants, including:
* Support of participants to understand and implement their NDIS plan and to build capacity and independence
* Review of support arrangements to understand each individual client and to provide an ongoing person-centred care plan
* Determine and work within the funding level of each participant, including support of participants to access further funding if required
* Support and management of plan review processes, including unscheduled reviews and internal complaints processes to ensure quality outcomes
* Advance planning for potential crisis management and advocating for change to promote client safety and wellbeing
* Delivery of timely person-centred supports
* Facilitate effective working relationships and open communication with participants, carers, family, external services and other key stakeholders (including but not limited to community organisations, NDIS and non-NDIS service providers, government organisations)
* Develop and maintain strong relationships across the sector to ensure participants choice and control
WHAT YOU CAN EXPECT FROM HIVELY
* Flexibility – full-time and part-time employment available (4 or 5 days per week preferably) with the ability to work flexibly from home

You might be an experienced Coordinator or someone starting their career in Support Coordination - we provide the skills and development opportunities to ensure you thrive in the role.

Qualifications
Qualifications aren't required for the role, however relevant industry knowledge or experience is desirable.
What we need from you
In order to visit your participants, successful applicants will need:
* Current Class C Driver's License
* Reliable access to a roadworthy vehicle with current registration and insurance
