As a Clinical Care Coordinator, you will play a pivotal role in our organization by leading clinical and care teams to provide exceptional care while fostering an understanding of each resident's unique identities and experiences.
About the Role:
The Clinical Care Coordinator ensures comprehensive clinical and specialized nursing care is provided, documented, and tailored to each resident's needs by our clinical staff. This involves prioritizing residents and their families in decision-making, considering their preferences, independence, dignity, and identified care-related risks.
Key Responsibilities:
* Ensure comprehensive clinical and specialized nursing care is provided, documented, and tailored to each resident's needs by our clinical staff.
* Prioritize residents and their families in decision-making, considering their preferences, independence, dignity, and identified care-related risks.
* Oversee and conduct all scheduled AN-ACC reappraisals, including mandatory and voluntary assessments as required.
* Hold Registered Nurses (RNs) and Enrolled Nurses (ENs) accountable for daily case reporting, care planning, and addressing patient concerns promptly.
* Conduct routine clinical assessments to address identified gaps in care.
Requirements:
* Current registration as a Nurse with AHPRA, with no restrictions.
* Senior management experience in the aged care sector.
* In-depth knowledge of the aged care industry, with a keen eye for emerging trends and best practices.
* Proficiency in managing human resource matters, complemented by strong negotiation skills.
* Skilled in utilizing various information technology tools and software, including Word and Excel, with the ability to adapt quickly to new skills as required.
