About the Role

We are seeking a compassionate and organised Service Assistant / Case Manager to deliver client-centred support and coordination services for SIL and community participants. In this role, you will work closely with individuals and families to assess their needs, develop tailored support plans, and connect them with appropriate services to enhance their well‐being and independence.

Key Responsibilities

* Conduct comprehensive client assessments to determine needs and risks.
* Collaborate with clients to develop, implement, and regularly review personalised case or support plans.
* Provide coordination for SIL and community participants.
* Support clients in navigating complex service systems and accessing essential resources.
* Undertake risk assessments and develop safety plans to promote client well‐being.
* Maintain accurate, timely, and confidential case records in line with organisational and compliance requirements.
* Build and maintain strong, respectful relationships with clients, families, and stakeholders.
* Apply strengths‐based, culturally sensitive, and trauma‐informed approaches in all client interactions.
* Assist clients in areas such as housing, financial stability, health access, education, and employment pathways.

Skills & Experience

* Relevant qualification in Human Services, Social Work, Community Services, or related field (minimum Certificate IV with demonstrated experience).
* Experience in case management or service coordination supporting vulnerable or diverse populations.
* Ability to conduct holistic assessments across areas including health, housing, finances, and social well‐being.
* Demonstrated experience supporting clients with complex needs, including mental health and/or domestic and family violence.
* Strong understanding of culturally and linguistically diverse (CALD) communities and inclusive practice.
* Excellent communication and interpersonal skills, with the ability to build trust and rapport.
* Ability to manage competing priorities and maintain accurate documentation.
* Multilingual skills are highly regarded.

Role Requirements

* Valid Driver's Licence.
* NDIS Worker Screening Check.
* Working With Children Check (Blue Card).
* First Aid & CPR Certification.
* Full working rights in Australia.
