This specialist nursing role supports older residents living in public and private Residential Aged Care Facilities (RACFs) to receive timely, high‑quality clinical care in their place of residence, reducing avoidable hospital presentations. The RIR Nurse Consultant works closely with the RIR Lead, At Home Program Nurses, RACFs, General Practitioners, and acute and community health teams to deliver coordinated, evidence‑based care. You will provide early intervention for acute illness, support chronic disease management, recognise and respond to clinical deterioration, and play a key role in care planning, transitions, and discharge from the RIR program. Key Responsibilities * Provide specialist nursing care to residents referred to the RIR program * Undertake triage, intake assessments, and comprehensive clinical reviews * Recognise and respond early to acute deterioration * Develop, implement, and review individualised care plans * Work collaboratively with RACFs, GPs, acute and community health teams * Support transitions of care and safe discharge planning * Advocate for resident‑centred, high‑quality care and optimal health outcomes * Promote health education and support residents and carers About you You will be a skilled and compassionate nurse who thrives in collaborative, autonomous practice and is passionate about delivering care in alternative settings. You will have: * Current registration with AHPRA as a Registered Nurse * Demonstrated experience in aged care, acute care, community health, or a related field * Strong clinical assessment, triage, and care coordination skills * Excellent communication and stakeholder engagement abilities * A commitment to person‑centred, evidence‑based practice * Ability to work collaboratively across multidisciplinary teams Postgraduate qualifications and experience in gerontology, community, or acute nursing will be highly regarded.
