Elka Home Care is seeking a Junior Care Manager to support the coordination of in-home aged care services for older Australians.
This role is ideal for someone with experience in aged care, community care or support coordination who is looking to step into a care management role. You will work alongside experienced Care Managers to support clients, coordinate services and ensure high-quality care is delivered.
Key Responsibilities
* Assist in managing a portfolio of Home Care Package clients
* Support care planning and service coordination for in-home supports
* Maintain regular contact with clients and families
* Assist with client reviews and service adjustments as needs change
* Coordinate support workers and service providers
* Maintain accurate case notes and client records
* Support compliance with aged care standards and reporting requirements
* Work with senior team members on client escalations and service issues
About You
* Experience in aged care, disability or community services
* Understanding of Home Care Packages or community care services (preferred)
* Strong organisational and communication skills
* Compassionate and client-focused approach
* Eager to learn and develop within aged care services
