At Uniting SA, we're seeking a dedicated and compassionate Physiotherapist to join our growing Community Aged Care team based in West Lakes. In this rewarding role, you'll provide person‐centred physiotherapy services to older individuals, empowering them to maintain mobility, confidence, and independence within their own homes across Adelaide Metro area.
About the Role
As part of a collaborative, multidisciplinary team, you'll play a key role in delivering high‐quality care to older people living in the community. You'll provide clinical assessments, develop personalised treatment plans, and offer practical interventions, all within the comfort of the client's home.
With the support of your Allied Health Lead, your key responsibilities will include:
* Conducting physiotherapy assessments and developing individualised treatment plans
* Providing education and advice to clients and their families
* Coordinating with equipment providers and other healthcare professionals to ensure safe, timely solutions
* Supporting clients in achieving reablement goals and enhancing their independence
* Identifying risks and implementing strategies to promote safe living at home
* Contributing to falls prevention initiatives and promoting healthy ageing
* Preparing clinical documentation and reports
* Collaborating with Care Partners, Nurses, and other Allied Health professionals to ensure holistic care
About You
We're looking for someone who is client‐focused, adaptable, and confident, with strong communication and organisational skills. Your passion for aged care and experience in delivering physiotherapy services in a community setting will be key to your success in this role.
To be successful, you will need:
* Tertiary qualifications in Physiotherapy
* Current AHPRA registration
* First Aid Certificate
* A valid SA Driver's Licence, reliable vehicle, and comprehensive insurance
* NDIS Worker Screening Check
* Experience in community aged care is advantageous but not essential.
* Knowledge of Home Care Packages (HCP), Commonwealth HomeSupport Programme (CHSP), and Transition Care Programme (TCP)
* Excellent communication and clinical documentation skills
* The ability to work independently and as part of a multidisciplinary team
