We have an exciting opportunity for a Clinical Assessor to join our team in Gippsland, VIC region. This is a community-based role allowing you the prospect to travel throughout the region to work collaboratively with clients both in their homes and in Residential Aged Care Facilities. You will use your clinical skills and knowledge to conduct assessments with residents and clients.
We offer a full-time permanent contract, working Monday to Friday - no weekend or shift work required.
To ensure you feel confident and supported in your role, we provide a comprehensive two-week training and orientation program. The first week is in-person at our Coolangatta office with travel, accommodation and meals fully covered as per our travel policy. Our next orientation intake begins 10th November 2025.

The role:
- Working independently within your region, utilising your assessment experience to conduct holistic style assessments with a client centred approach focusing on wellness and reablement
- Capturing individual client information and working collaboratively to develop their support plan and unique goals
- Supporting older Australians to continue living independently, by considering supports such as assistive technology, social support services, and community care.
- Utilising the integrated assessment tool with My Aged Care
About you:
- Experience in assessment and goal orientated support planning in aged, community, health or social services
- Confident communicator with a positive approach when collaborating with clients, families, staff, and other stakeholders
- Local knowledge and the ability to network with stakeholders within your region
- Advanced IT skills with previous experience working across multiple platforms (Microsoft Office Suite)
- Previous experience working with KPI's to ensure high quality client outcomes
- Ability to manage your time effectively in an autonomous working environment
What to bring?
- Tertiary qualified Registered Nurse, Occupational Therapist, Physiotherapist with 5+ years of experience.
- Minimum 5 years’ experience within aged care
