About your role:
- Deliver high level clinical pharmacy services within the Community Care team within the CHHHS in accordance with prescribed professional and ethical standards with infrequent need for direct clinical practice supervision.
- Contribute to the development of clinical practices, procedures that support the continuum of care engaging cross-discipline referencing where relevant in First Nations and Rural and Remote Care.
- Liaise with patients and their families, other health professionals, supervisors, students and researchers, as required to ensure continuum of care.
